These Terms of Service ("Terms") govern your use of the SDP Enrollment Tracker (the "Service"), operated by Self-Determination Institute ("SDI," "we," "us"). By creating an account, you agree to these Terms.

What this Service is

The SDP Enrollment Tracker is an informational tool that helps participants and support people track progress through California's Self-Determination Program (SDP) enrollment process. It is not affiliated with, endorsed by, or operated on behalf of any Regional Center or the California Department of Developmental Services. Projected dates are estimates only and are not a guarantee of when any step will actually occur — always confirm requirements and timelines with your Regional Center.

Your account

You're responsible for keeping your login credentials secure and for all activity under your account. Let us know right away if you believe your account has been accessed without your permission.

Support person access

If you're a participant, you may invite a support person to view (but not edit) your progress. You control this access and can revoke it at any time from Settings. If you're a support person, your access is limited to what the participant has shared with you, and can be revoked by them at any time.

Communications

If you opt in, we'll send you reminders about your own enrollment steps by email, text message, and/or in-app notification, based on the preferences you set. Message and data rates may apply. Reply STOP to unsubscribe from text messages at any time. We do not share, sell, or provide your mobile phone number or messaging consent data to third parties or affiliates for marketing or promotional purposes. Acceptable use

Please use the Service only for its intended purpose — tracking your own (or, as a support person, an invited participant's) SDP enrollment. Don't attempt to access another user's account or data without authorization, upload unlawful or harmful content, or interfere with the Service's normal operation.

No warranty

The Service is provided "as is," without warranties of any kind. We do our best to keep the enrollment checklist and projected timelines accurate, but SDP requirements and processes can change, and this tool may not reflect the current requirements of every Regional Center. Always verify with your Regional Center before making decisions based on information in this app.

Limitation of liability

To the fullest extent permitted by law, SDI is not liable for any indirect, incidental, or consequential damages arising from your use of the Service, including delays or errors in your SDP enrollment process.

Changes to these Terms

We may update these Terms from time to time. If we make material changes, we'll update the date at the top of this page.
